This is blissfully simple. On contacting the practice you may wish to discuss your needs with my staff to confirm that we are the right practice for your you.

As soon as you have made an appointment, we will send you a "Welcome" pack which contains much useful information. if your appointment is only a day or two after your telephone call we will hold this pack for you to collect when you arrive. In this case, please arrive 10-15 minutes before your appointment in order to complete our medical / dental questionnaire.
